I, _________________, a participant in the Changing Lives Through Literature Program of this court, agree to the following conditions:

(1) To attend ALL scheduled sessions of the CLTL Program.

(2) To be punctual for ALL scheduled sessions.

(3) To bring assigned texts for their respective sessions and to maintain said texts while they are in my possession.

(4) To read assigned materials PRIOR to the start of each session and to be prepared to discuss said materials with other program participants and/or facilitators.

(5) To provide verification of employment and residence at each session.

(6) To comply with all conditions, both general and specific, of my probation.

(7) To treat all program participants, facilitators, and guests with respect.

(8) To refrain from any conversation with Judge Reardon or any visiting judge(s) pertaining to any former, current, or pending legal matters involving myself.

I understand that subsequent charges and/or failure to comply with any of the conditions listed above will result in dis-enrollment from this program and the return of supervision to my primary Probation Officer with appropriate recommendations.

I understand that successful completion of this program MAY result in the recommendation for a reduction in my respective sentence.
